We are looking for a confident and skilled individual to join our friendly and dynamic Reablement team covering the Rayleigh and Castle Point areas. The Facilitator role involves line managing a group of Domiciliary Support Assistants, completing first visits, assessments and reviews of service users within their own homes, rota planning and writing support plans for the Domiciliary Support Assistants to help Service Users achieve a higher level of independence.

Knowledge of domiciliary care practices is a big advantage, along with a level 3 NVQ if possible. All training will be provided by the department. You will need to be a car driver as the post will involve you visiting service users around the area. You will also be required to work various shifts across the week, to include earlys, lates, weekends and bank holidays.
